Client Rights

At Championed Minds, every client has the right to receive services in a safe, respectful, and supportive environment. We follow all requirements outlined in the Ohio Administrative Code (OAC 5122‑26‑18) regarding client rights and grievance procedures.

Your Rights Include:

Dignity & Respect

  • The right to be treated with dignity, respect, and consideration at all times.

  • The right to receive services free from abuse, neglect, exploitation, or discrimination.

Participation in Care

  • The right to participate in the development of your treatment plan.

  • The right to be informed about the benefits, risks, and alternatives to services.

  • The right to refuse services within legal and safety limits.

Confidentiality

  • The right to privacy and confidentiality of your records, as protected by state and federal law.

  • The right to be informed of the limits of confidentiality.

Access to Services

  • The right to receive services in the least restrictive, most appropriate environment.

  • The right to receive information in a language or format you understand.

Safety

  • The right to receive services in a safe, clean, and supportive setting.

  • The right to be free from seclusion or restraint unless absolutely necessary for safety.

Grievances & Concerns

  • The right to file a grievance without fear of retaliation.

  • The right to receive a timely, fair response to any grievance.

  • The right to contact outside oversight agencies if concerns are not resolved.
