Grievance Procedure
Championed Minds is committed to ensuring that every client and family member feels safe, respected, and heard. If you have a concern about your services, you have the right to file a grievance at any time without fear of retaliation.
This procedure follows the requirements of Ohio Administrative Code 5122‑26‑18.
How to File a Grievance
You may submit a grievance verbally or in writing. Grievances may be filed by:

Parents or legal guardians

Authorized representatives

Staff members on behalf of a client

What Happens After You File

Your grievance will be reviewed within 3 business days.

You will receive a written response within 10 business days.

If additional time is needed, you will be notified in writing.

You will not face retaliation or discrimination for filing a grievance.
